The Long Application Form is an online-form. We recommend that you review the questions first before filling in the answers. Alternatively you can print out the form and any popup info page you might want, to enable you to gather all the information at your own convenience.

De Long Application Form is een online formulier. We raden U aan om de vragen eerst globaal door te nemen voordat U ze gaat beantwoorden. U kunt ook het formulier uitprinten, net zoals de popup informatie pagina’s wat U in staat stelt om alle informatie eerst rustig te verzamelen.

Als u sommige vragen nog niet kunt beantwoorden vul dan niets in, verstrek alleen voorlopige antwoorden als u redelijkerwijs denkt dat de antwoorden waarheid zullen worden.
